Photo ID for person applying

Birth Certificates for all teens

Report Cards for all teens 17 and 18

Please remember that all applicants will go on a waiting list. All applications are pulled from the waiting list in the order they were received. The number of teens accepted are based on the number of donations received. If you would like to inquire about becoming a business drop off location, please contact us at giftsforteens@yahoo.com.
